The short answer is yes.
However, she does not die on the Titanic, but after a long and full life, she dies in her sleep on the research and salvage ship that is trying to find the necklace. Upon her death, she is returned to the love of her life (Jack) and in the place that she knew him (Titanic) and at the age she knew him, i.e. younger.

No, the movie is told from her perspective as a flashback. She is a very elderly woman telling the story... I do believe she dies of natural causes at the very end of the movie, but she did not die when the ship sank. Jack did.

Yes she does. Remember when she's floating around and she's back in the boat again and she sees all the people that died and meets up with Jack at the clock? So, yeah, she dies. When I figured it out I cried even harder.

Yes she does. She died in her sleep and rejoined Jack and all the others who died. As if watching all of the horror of those poor people dying wasn't enough, they had to go and kill her too.
