97 e250 - 5.4 windsor. Spark plug blew completely out of head, shredded the ignition coil, stripped out the threads in the head

The only explanation is that the plug was not torqued correctly to the head, probably was loose. Most auto part stores carry a spark plug hole re-thread kit to re-thread the cyclinder head for a plug; not too expensive. Be sure to tighten plugs to specifications.

Someone, at some time, has stripped the threads in the head. On the Ford full size vans equipped with a small block, some of the plugs can be difficult to replace.

Now call your local dealership and threaten them with a small claims lawsuit (there are a lot of cases where these were won, haven't seen one yet that was lost) it might help you get some relief from Ford.

a bad spark pug or maybe it was not in good and tight and got loosed and while holding on my a couple of threads blew out

Don't catch the news much do you? This engine is notorious for this problem.

poor design.. a few cylinders have very few threads in the plug hole... very common.... i feel for you
