Well you are definitely heading in the right direction just asking for help!! A+ for you!!!
Laziness is a mental issue - you need to get your mind set to motivated and not lazy. Tell yourself to get moving when you feel like sitting around doing nothing. Just set a goal and say if I do this and get it done - I can watch tv or whatever it is you do. You'll realize after awhile that you really aren't missing much on the tube or computer and you'll feel better getting stuff done!!!
Disorganized (tough for me too) - get yourself some folders and mark them for each class. Have a couple at home in your bedroom (or wherever you do your work) and mark them work to be done, notes, etc. Whatever is going to help keep you organized.
Forgetfulness (another killer for me) - thank God for post it notes!!! The minute you think of something write it on a sticky and put it on your bathroom mirror or on the back of your bedroom door, wherever you will see it EVERYDAY to remind yourself.
Finishing assignments on time - refer to advice above and you'll have them done on time!!!
Basically, it's just a mindset thing, set your mind to do all these things and you WILL achieve your goals! And you'll be better equipped for high school and college!!!! Don't beat yourself up, sounds like you are doing pretty ok grade wise.

You are part of the way to improving your academic record already. You gave the reasons, now focus on fixing these problems.
First, get a small notebook, notepad, and start writing down things you are not supposed to forget.
Second, make a plan for your day. School is already settled, so don't worry about that. It is the after school time you need sort out. Plan on and really do spend time on your studies. Take your worst subject (math) and allocate more time for that than the other subjects as you want to get that up the the B level. Do not kill yourself though. Schedule less than three hours of study and you probably won't need that.
Do not listen to music or have any other distractions when you are studying. Studying is what you are doing, nothing else. Focus.
Third, when you are given an assignment, make a short list of what you have to do to get it done. Write down when you will do each of these and then do them.
Organization seems to be your biggest problem. B grades are pretty good, remember that 62% get C (the "Bell" curve, remember that?), so you are already in the upper 15%. With a little better organization you can solidify that position and possibly move in the A level.

Good for you ! Good for you for wanting to do better in school.We should all want better from ourselves. So you have written down the things that are holding you back. Good start. Now work at changing one thing at a time. You say that you are forgetful...change that by writing everything down and use the information that you have written.Eat healthier because it helps you to improve your thinking. I know that this may sound impossible but don't wait until the last minute to begin an assignment. As soon as you are given an assignment, start it. Also when you get several assignments that are due nearly the same day...focus on one assignment at a time and complete it before going on to the next.Spend weekends preparing for the upcoming week. You can do it.

Good for you for recognizing it.
I did horrible in high school and undergraduate school. I had to really fight hard to keep a 3.8 average in graduate school.
Me? I was diagnosed with A.D.D. I am not hyper, I just get really overwhelmed easily by stuff.
Even without the diagnosis, you can use a lot of A.D.D. strategies to help you get a handle on things in school.
I taught school for about 20 years. And I continue tutoring kids of different ages. Some of the more helpful strategies I have used on kids like "us," are as follows:
You KNOW you want to improve, so there's a great goal.
1. Set up your homework or study area in a place with the least amount of distractions.
2. Set a timer for about 30 minutes. Then work steadily until the timer goes off.
3.When the timer goes off... take a break. A meaningful break. GO for a walk. Eat a sundae. Listen to music. Work a puzzle. Call a friend. BUT! Set the timer for your break, as well.
The timer works very well for me.
Another thing... get some file folders... decorate them and make them pretty... and have TWO that are strictly for each subject.
One is for finished work. The second is for works in progress.
I will also send you links to places that help folks like us. I encourage you to check them out, as they often provide nifty ideas for helping you get and stay organized and "on task."

all u need is a little bit of encouragement and motivation.
try hanging out with some smart people at class. this way, when they do their work, u feel like u dont want to be left out and u actually get some work done.
if u are up late cramming for ur exams, try listening to music. this way, u dont get bored and fall asleep. set a goal. for eg, a grade higher for maths, and try working for this. dont try to set up unrealistic goals that u can never get done. u could talk to the guidance officer and get some tips as well but experiment on different methods to study until u find the right one.
the best thing for forgetfullness is writing down all the stuff u need to get done on a piece of paper and keep this in ur pencil case (so that u dont forget where u kept it!). time tables at home help as well. oh, and before ur exams, make notes and write them over and over again and do a lot of reciting.
hope this helps! it does seem like a lot of hard work, but at the end of the day, when u look at it, it's all worth it!! u dont want to be nobody, right?!

First off, be consistent, not lazy! You don't have to be continuously berating yourself to be successful in school!
If you are forgetful and unorganized, visit your school academic counselor (or trusted adult) and have them help you organized your folders/notebooks by class and color. Adding an agenda (if not already required by your school) is likely essential for success to help with your memory. Write ALL assignments and due dates in your agenda/planner. Make sure you note in the days before an assignment is due what you need to do. For example, if an assignment is due Friday, split it into pieces as needed throughout the week--Monday=research, Tuesday=compile research, Wednesday=outline and rough draft, Thursday=rewrite and polish, Friday=turn in!

first thing you need to do: GET AN AGENDA. this is crucial. in it, write all of your school subjects and any assignments you have to do, and also write the due date. this is extremely important and it will help you so much. as far as being lazy, you're just going to have to push yourself. give yourself a small reward every time you finish an essay or project. it's great that you want to improve, but you're going to have to motivate yourself. it might also help if you get a group of your friends together and form a study group. i know that helped me. but honestly, an agenda will work wonders. it will keep you more organized and help you remember assignments. good luck!

Have you been tested for learning disabilities. This doesn't mean your dumb, but it does implicate that you are having problem keeping your attention focused. Those ADD you've been hearing about might be you. Its an attention deficit disorder. Or may be ADHD, (attention deficit hyperactive disorder.)
I'd like to make a suggestion: go to your local dollar store and buy a pair of sunglasses with blue lenses. Wear these when you read or do your homework, if things improve, then you know that its a "L-D" problem and you'll learn to adjust to it.
If you're still having problems, go to your guidance counselor and ask her can they test you for ADD or ADHD

I notice that most of the time its not the geniuses that have the best grades, its the people that work hardest. You can be the smartest person in the world, but if you dont apply your knowledge its useless. Just work hard and you'll be fine.

same thing for me....i tried organizing more. School is mostly based on homework and test. So do all your homework and classwork and try your best on test. School is not really that hard if you just focus and get things done early...keep a schedule if you have to. Hope you better your marks. =)
