For the first time, yes, they have to spell out their name. However, if two individuals communicating by sign language are going to be 'speaking' with one another on a regular basis, they will derive handles for one another - some easily formed, unique sign that refers to the other individual. Sometimes certain letters will be used instead of making up a unique sign - for instance, referring to someone names J-E-S-S-I-C-A by using the sign for J.
