The hole is there so the server/busperson going in/out of the kitchen can look for any "traffic" that might be on the other side of the door

They are swinging doors usually and it makes it possible for the servers to see if it is clear. Round so it is smaller and dinners wont be bothered by all the commotion that goes on in the kitchen .

So you can see who is coming or going so you don't get knocked over or drop your plates

you don't want to knock anyone coming the opposite direction so you use it like a window
