your car insurance (ring the company)
your car's registration document

fill out the form on the back of the registration document and send to dvla. do the same with your driving licence

Your driving licence and registration document have instructions on them. However there are clearer instructions available on the DirectGov website.
http://www.direct.gov.uk/en/Motoring/OwningAVehicle/ChangesToYourRegistrationCertificate/DG_4022055 for registration
http://www.direct.gov.uk/en/Motoring/DriverLicensing/NeedANewOrUpdatedLicence/DG_4022088 for driving licence

You also need to inform your insurance company - call the phone number at the top of your certificate

If you have finance on the car you need to inform the finance company (see the finance agreement - if you cannot find it your bank should be able to help with contact details they have for the direct debit).

driver license, titles/pink slips, registration, insurance pretty much in that order.

if you stayed in the state you probably wont need to to get a new tag other than that you have 30 days to change all your addresses(in NC but I would guess its probably right about 30 days everywhere)

Look on the documents.
Somewhere you will find the information you require.
