Oh, I absolutely was disgusted by that Dahmer movie- the whole biographical thing- scary and reprehensible.
I think the unedited Exorcist where Linda Blair runs down the stairs like a spider was pretty grotesque too.

I'm not certain about THE scariest, (I've seen many horror films), but one of the scariest is in the original Friday the 13th: Almost at the very end after Jason's mother is already killed, and Alice is sitting in a boat and this relaxing music is playing, and you think it's all over, then, Jason jumps up out of the water...unfortunately, it was only a dream.

In high tension when the killer brings the father's head through the stairs bars and slams a dresser into it, squishing his head with this horrible crunch, while some of his head is taken off and the neck keeps squirting blood everywhere. Pretty Intense...

Zombi 2
A zombie breaks through a door, and grabs a woman by the throat. Then, he slowly pulls her towards him. However, there is a piece of splintered wood jutting out of the broken door, and is slowly approaching her eye. Then, with very few cuts/editing, you see it actually pierce her eyeball through!
disgusting, edgy stuff in that'n!

Aliens -- when the space marines are scouting through the biolab, inspecting the face-huggers in the sample tubes -- and one twitches.
Alien had some shocker moments as well.

In 'Alien' when the creature is wrapped around the guys face and is inserting an egg into the guys stomach. (I think that is even scarier and creepier than when the alien pops out of his stomach later in the movie!)

ohhhhhholy crap!! the part in 'Frankenstein' where the Monster comes in through the door backwards!!! and before he comes in, His footsteps fade in in the background, just the noise was creepy!! that whole scene scared the crap out of me! for 3 months a picture of Him would scare the crap out of me!
