it really depends upon the players you have on defense. is it a hard hitting defensive team or is it built on speed and swarming around the ball carrier?
the 46 defense is most famous for holding the most dominant defense in nfl history, the 85 bears. it is still used once in a while, even by the chicago bears today. however, most teams dont prefer it as it lacks great defense against powerful passing offensive teams.
the 4-3 is the easiest defense to assemble in the nfl. it is the most basic and if the right pieces are assembled correctly, it can both provide an effective pass rush and defend against the ground and air.
the cover 2 became very popular from the buccaneers super bowl run, which i believe was in 2002. it is relied on speed, and is effective in getting turnovers (interceptions). however, it has a huge flaw when it comes to deep routes near the sideline.
the 3-4 defense is the most difficult defense to assemble. the teams that can make it work correctly are the teams that dominate every year. it allows the defense to confuse the opposing team's qb's more frequetly with the diverse defensive plays of the linebackers.
-that's great that you get the 3-4 to work in madden. that will give you an ultimate edge. i tend to turn the ball over a little more often against 3-4 schemes as opposed to 4-3 schemes as they are more complex.

Well naturally, Madden is the end all debate in coaching questions....
I always use the 4-3 in madden. It's better against the run than the 3-4 and the cover 2, and if you have a good line, you can get a great pass rush. The 46 is too easy to beat deep, so I don't use it too much. Obviously, you will struggle to a certain extent in coverage, but if go into the nickel and dime on obvious passing situations, you'll be fine provided you don't call risky blitzes too much while in 4-3.

I personally think the 4-3 is the best formation overall. As far as blitzing I think that the 3-4 is the best because you don't know where the pressure is coming from. Its also really good because you can run a QB spy with one of your defensive lineman and blitz with the LB. Cover 2 is good against the pass especially if you have good coverage linebackers like Urlacher and Ray Lewis but it leaves a big hole over the top between the safeties. Against the run the 3-4 is the best formation. there are 4 linebackers which means that its harder to run up the middle. It can also be used to stop runs to the outside by having the outside linebackers play the flat.

Cover Two is good at giving help over the top but isn't that great against the run. The 4-3 Is great for flushing out QB'sand getting pressure on passing downs. The 3-4 is the best becuase the 4 LB's can easily stop the run and Defend the medium routes. The Steelers have the best 3-4 in Madden 07 becuase of Porter, Haggans, Farrior and Foote.

Cover 2 isn't a formation. All it means is that two defensive backs (presumably the safties) each cover one half of the field deep. This gives you 5 guys to cover on a standard 4 man rush, either in a zone of man- to- man, and 4 guys to cover on a nominal blitz.
The 46 scheme really only worked with the Bears 85 defense, and that's because with their talent, anything would've worked!
The 4-3 and the 3-4 are the usual base defenses to build around. I prefer the 4-3 myself, but each to his own... the 3-4 usually works better against outside runs while the 4-3's usually better against inside runs, and in EA's games, usually inside runs are more effective then they are in real life.

It depends on the team. Some don't really have a good enough line to use the 3-4. They need that extra lineman with the 4-3. You really have to consider the TEAM before deciding what is the best scheme. Most are designed to play one style or the other, that is how they drafted over the years and how they are set.

3-4 It works for us though sometimes the Patriots use a 4-3!
As far as Madden, I guess it depends on how you set it up!

it all depends..
when they pick their play it shows how many WR's, TE's and RB's they have..
the more WR's..will tell you to go into the dime package
the more TE's and RB's usually means its going to be a run so you should go with nickel, 3-4, 4-3, whichever your team has
and if your not sure, go with a cover 2, and have maybe one man blitz just to be safe, just dont take him out of man coverage
