A woman gets pregnant by having sexual intercourse unprotected, whether its your 200th or first time, sexual intercourse is the same if you did it unprotected. There is no such thing as more or less likely the first time compared to any other time but if you are asking this question, it means that you are not knowledgeable enough to even be having sex so I hope you are not. Definitely possible. Anytime you have sex, you have the possibility of getting pregnant. There is no safe time during your cycle.

Yes, it is very possible especially if you don't use any birth control.
She needs to see her Gyn to find out her options and to get on the proper one for her.
Use a condom whether or not she is using anything. It will protect you both from STDs (not everyone knows they have them) and extra protection against pregnancy.
