Peter Max designed the cover art and George Martin was most likely behind the soundboard side two was arranged mostly by him.

I'm guessing Alan Aldridge - his work looks like Max's. Aldridge did an entire book of illustrated Beatles lyrics in 1969, the same year "Yellow Submarine" was released...

It appears it be Heinz Edelman. Peter Max never did anything with the Beatles.
