I have been told that they legally they must accept an offer of regular payment no matter how small. Is this true or can they reject my offer and start collections or judgement proceedings? What are my real rights and options?

They can start collections and garish your pay if you don't pay them everything you owe them. If they agreed to a payment plan they should stick with it.
Old wives tale is that if you sent them even a tiny bit you can't be sued but it isn't true. Think about it if you owed hundreds of thousands and had assets you would just sent them $5 a month until you died to get out of paying.
A garnishment could take up to about 25% of your pay or freeze a bank account so paying more might be cheaper for you.

How much is your bill? They do not need to accept any offer. An offer should be talked about prior to the date of service. If you did not mention about only paying $50 a month to them for the service, they have the right to send the bill to collections. I would suggest that you to get a loan and pay the bank $50 a month instead of oweing the hospital. If every patient is like you, the hospital will eventually close down.

Getting a loan to pay a medical bill is a terrible mistake. Never replace a medical debt with no interest and limited credit impact with a loan carrying high interest and a bigger impact on your credit!

If they are saying that $50 is not enough, then it means that your outstanding balance is relatively high. They may be looking for an amount closer to 2% of the balance. Try sending an amount higher if you can afford it. If they cash it, then they are implying that the payment is acceptable and you could continue sending in that same amount.

Legally, they don't have to accept any offer. If you send the payments, they WILL accept the money. It is VERY unusual for them to sue if you are making regular payments. If they suspect you could pay more, they still have the legal right to sue for full payment.

Well, I'm not sure legally what they must accept. I attempted to read through the link I'm attaching for you to review. Hopefully your answer is in here some where. This link is from the Federal Government so you can take anything you find in it and forward to you debtor in attempt to set up payment options with them.

I think that if you offer even a dollar they can't put a judgement against you. It is a good faith effort.

yes this is true as long as you pay them something, they can not put you up for collections, if you ever stop paying them they will start collections asp.
