If you can understand them, in the same wave length, know what they are saying to you, without actually having to get too deep into explaining how they are feeling or expressing.
Otherwise if you're actually touching them, I suppose they want to make sure it's you and not a ghost--feeling them... I would guess.

it means do you understand me, or do you know what I am going throw or how I feel, like when you love someone and he/she act like doesn't know how you feel, so u tell him/her YOU FEELING ME

from what i've experienced, it means "do you understand what i'm saying?"
thats probably what it means.
