There was a Great King in the Bible named Nebuchadnezzar over Babylon. He was stated to have become a freak of nature because of his arrogance against God. He stood on his palace balcony one day and realized his greatness, shook his fist at the heavens and said that there was no one higher then him, that he was god. That very instant an Angel of God came down and touched his mind. He became a mad man. He grew claws and feathers like a bird. He could not reason any more like a human being and he was put out of the palace by his armor bearers. He was sent into the fields and forest to graze grass with the cattle. Only when he became aware of God's presence, which took seven years, that He was God alone, then God restored him to his place in the palace and made him king again. This is an impressive story but the way he was done was appalling.

It means there is something about them that is not exactly like God makes most people. They could have a huge growth or deformity, not a defined sex ( a hermaphrodite), have no arms or legs or all 4 extremities be missing. There could be a wide variety of things, but the impressive thing is when they learn to function and have a positive approach to life with whatever the extreme situation might be.

It depends on the context - if someone said that about a highly skilled athlete, it could be interpreted as a compliment.
OTOH, if someone looked at a person's face and said that, impressively or otherwise, it could easily be interpreted as an insult.

People who live their lives as primates, work, breed, die, are natural. True Humans are freaks of nature in that we can rise above our emotional instincts. If you are called a freak, take it as a compliment.
