I live in the country, with the largest town being 45 miles away where I could visit a pet store, Wal-Mart, Petco, or a Petsmart.

Talk to a friend or relative who needs a hobby to start breeding them for u. That way you know where they are coming from; you can also do it yourself if your not bogged down by your existing fish since any place you purchase them can have a disease.

Frankly you can't. There is no where, not even the best shops that you can buy live fish with no chance of disease. The best thing for you to do is set up a second tank or some type of filtered container to hold feeder fish and use that to quarantine them before feeding them to your fish. This way you can buy in bulk, allow the fish to quarantine and be fairly sure you're not getting a disease in your tank and then feed them as needed.

Feeder fish always have the possibility of carrying diseases. The best way to insure they don't get your fish sick, is to quarantine them for a week or two and remove any sick/dead fish.

Try breeding your own feeders. That way your chances of bringing home a disease or parasite go way down.
