those are some of the things it asks for but what made me think this was a scam was the fact that it wants my email but it sent one to me? also if this was really yahoo wouldnt they already have my email?

It's a scam.

If it is in the bulk folder no doubt that it is a scam but if its in the inbox u can report to yahoo first. But most likely doesn't matter it is a scam. You'll get thousands of those offers. So yeah it is a scam.

Seeing the information that you have provided, It is a phishing site.,,.i.e,..This particular site is a scam!!!.Don't give away any king of information.And be careful while browsing on this page there is every chance a keylogging software may be active to catch your key strokes...........
