neuroscience has investigated this.
basically there are chemical firings of neurons etc in the brain. if enough are firing it makes it to the level of a thought.

The brain (or mind) is an active little unit and loves thinking about stuff and ‘doing‘ stuff. You can not ever please a mind absolutely – it always has a job to do and if you aren’t giving it what it wants, it’ll just make up shït and tell you it’s true.
(It reminds me of My Little Robot:
I have a little robot that goes around with me.
I tell him what I’m thinking, I tell him what I see.
I tell my little robot, all my hopes and fears,
he listens and remembers all my joys and tears.
At first my little robot followed my command,
but after years of training he’s gotten out of hand.
He doesn’t care what’s right or wrong, or what is false or true.
No matter what I try now, he tells me what to do.)
The thought comes as a desire for attention in response to another thought which was, for any reason, discarded.
The thought that won your 'attention' attracts other supporting data - depending on its importance it will stay and be acted upon or discarded like thousand of thoughts per day are.
If 'you' decide (or you 'decide') to act on it the thought will gather even more supporting info (for instance, previous experiences with the end product) and you may right it down or draw it or just make the toast and eat it.

Thinking is a spontaneous process, and since this is the Philosophy area I will use a quote from Socrates: "Know Thyself." And strangely enough, a person has to move beyond the thought to know thyself.
So the process of understanding thought is that of finding what is behind the thought. Then, and only then, can you really know where thoughts come from.

I would assume its the same for everyone, we try to systematize knowledge gained by observation, study and experimentation to come up with principles, facts, and valid hypotheses.
