There have been so far a total of 119 space shuttle flights, with a total crew complement of 820, according to Wikipedia. Of those 119 missions 2 have resulted in the loss of crews, seven astronauts in each case, making a total of 14.

Crunch those numbers and you come up with a 98.3% success rate for shuttle flights, whether you do it based on number of flights or number of astronauts.
