Bone pain, proximal muscle weakness, chronic muscle pains ('fibromyalgia is often undiagnosed vitamin D deficiency), ataxia and/or frequent falls. Vitamin D deficiency is now at epidemic levels. Acute symptoms of deficiency are fairly rare because deficiency has to be fairly severe before acute symptoms develop. Acute, severe deficiency leads to rickets/osteomalacia.
Chronic problems associated with vitamin D deficiency include: osteoporosis, increased risk for certain cancers, increased risk for autoimmune diseases such as multiple sclerosis, impaired immune function, impaired central nervous system function, and impaired muscular functions.

The biggest sign I have seen besides bone pain is depression. D3 is really good for that. Hope this helps!
