No one knows for sure, but there are many theories out there. Some astronomers believe the moon was a captured small planet, but most astronomers disagree.
Another theory is that the moon and earth were formed at the same time, out of the same gas and dust. The same elements such as iron, aluminum, and titanium are found on both the earth and moon.
But still another theory, first proposed in the 19th century suggests that the Earth was a rapidly rotating molten body that split in two. The moon was the smaller of these two bodies.
A different theory states the moon was formed from a ring of fragments that had evaporated or been detached from the Earth when it was formed.
Some belive that a giant planet hit the Earth. This violent impact threw a cloud of fragments into orbit around the Earth. They eventually recombined to form the moon.
Astromers do not all agree on all the theories, they do agree on when it was formed. The oldest rocks from the moon were about 4.6 billion years old. So the moon is about the same age as the Earth.

The first answers are correct as the most widely accepted view of the Moon's formation. A Mars-like object collided with the still forming Earth. It is mostly known as the Giant Impact Theory. You may confirm that it is the most likely explanation by googling it for yourself. Here is one site.

The most widely accepted theory is that an object about the size of Mars impacted Earth not long after it formed. A huge amount of debris from this impact was blasted into space where it at first took up orbit about Earth (...Earth with rings!). Gradually all this material coalesced into a sphere that became our moon. At that time the moon was only about 14,000 miles from Earth, but it's been receding ever since, currently at a rate of about 1.5 inch/year.

Another theory about the moon states that 2 moons were formed. And after a few thousand years they caught up with each other, collided, and formed the moon we know now.

As Earth was forming and space was a mess a large meteor hit the Earth. Chunks of the Earth was thrown out into space. Over a period of time the larger chunks were drawn together by the force of gravity. Eventually more and more pieces were attracted to the neo-moon as it headed away toward Earth until it reach the size that was close to what it is not. The force of the Earth's gravity in combination with the gravitational forces of the other forming planets left the Moon in its present position.

There are different theories, but one of the most popular is that during earth's early life, 3-4 billion years ago, something very large smashed into the earth causing all kinds of debris to fly into space. Gradually the debris gravitationally bounded and formed the moon.

According to Absolute Motion Theory,the earth and the moon were formed as a two planet system, over the same period of time, by a gradual buildup of matter from the same basic cloud of solid particles. These particles gradually expanded into one another until the earth-moon system was formed.

i think the latest theory is that when earth was starting to form, it collided with a mars-sized object, some of the object was added to the earth, some flung into space, and some was eventually pulled together to form the moon.
