hard braking does not affect mpg.... it is the fact that now you need to accelerate the mass of the vehicle back up to speed to keep up with the flow.. so anticipate the flow and use the rolling mass of energy to get to the next mile marker without wearing out your brakes for no reason

Sudden braking on a regular basis will decrease your fuel economy, as you are pressing the gass longer, if you brake in a short distance, then you have been keeping on the gas longer, if you let off the gas sooner and slowly bring yourself to a stop, you are traveling the last bit with the vehicle only at idle and not pressing the gas, over time this will add up and affect your mpg slightly.

No, but sudden acceleration and on and off acceleration do.
