Probably the easiest and most cost effective way to handle this, is to hire a contractor to come in and remove it for you. It would only be more cost effective, if you owned the equipment to do this... since you are asking about it, I assume you do not.
A contractor with a skidsteer (aka BobCat), can use a 'jackhammer' attachment on his tractor to break up the concrete into smaller chunks, and use the skidsteer to scoop it up and haul it around your house, if necessary, to a waste container. A contractor can most likely do this in a day, and lay sod, or seed the ground for you. The "skidsteer", or "bobcat" tractor is small enough to be able to work between most homes, very easily, without doing too much damage to the yard.
If you feel adventurous, you can rent one of these tractors from a rental store, and do the work yourself, tho, you should expect it to take a bit longer than someone familiar with the equipment.
If the old concrete is pounded up into gravel size chunks, it can even be used as fill for driveways, patios, etc.
Good Luck

Make a mountain of calls until you find someone to do it for you cheap. Or the old fashoin way, a sledge hammer. and When you begin hitting this it will bust into a hundred small peices at times feeling as if you been shot with 10 bb guns. make sure you wear safety glasses.

Yeah, hire some day laborers cheaply and have them break it all up with sledgehammers and stack all the broken concrete up in a pile. Then you hire someone to cart it off for you. No matter what it costs it will be cheaper than doing it yourself and your back will thank you for it later.

Get a sledge hammer and go to work;
2nd answer; Get several sledge hammers and invite the neighborhood kids over to have a few whacks at it.
Just not all at the same time or you may have some broken heads when they really get into it.

Don't waste your time with a crappy sledgehammer. Go to a equipment rental store and rent a Partner saw with a diamond blade on it. That way you can saw it in small squares and move them with a wheelbarrow or dolly.

yes, with a sledge hammer. but it is a HUGE pain in the butt. I recommend you rent a jackhammer. A sledge hammer will take days, a jackhammer will take hours.
