For Core 2 Extreme X6800, it appears to differ only in terms of its 2.93Ghz speed and higher TDP of 75 Watts vs 65W for Core2 Duo.

For Core2 Extreme QX6xxx, they are the quadcores on the Kentsfield core. TDP is 130 watts.

The Extreme processors model starting with QX have a 8mb cache and Quad core and the X series has 4mb cache and Dual core. The best way to find that out is going to the Intel website which I linked below. (I am guessing you are comparing the Intel Core 2 Duo to the Intel Core 2 Extreme and not the older Pentium 4 Extreme)

p.s. The second link is a link to the Intel compare site

Look on the intel processor site and click on the 2 different processors and then click on the specification tab this will show you the difference. a big factor is the price, the extreme processors are for professional gamers who do this for a living and they come with an extreme price tag and usually have a FSB of 1333 Hz which means you need a fairly up to date mother board with that capability.
