Coldest is definitely the killing of Fredo. He waits for his mother to die before enacting revenge on his own brother. That's cold.
Cruelest is the decapitation of Khartoum.
Sickest is the look on Luca Brasi's face as he is being strangled to death and held in place with a knife through his hand.
