If you get off at an "exit" there is also an on ramp to the highway nearby (usually just across the road that you exited onto). An "exit only" means there are no on ramps to get back onto the highway.

Exit means that you can exit from that lane! Exit only means you MUST exit from the lane!
Normally when the sign says EXIT, you can take the exit or continue. With an EXIT ONLY lane the lane will become the exit and you have no option!

Exit only means that if you stay in that lane you have to exit. Exit on the other hand means that you can exit or continue on.

exit only means that you must exit if you are in that lane. exit itself means you can stay on the highway as the lane continues beyond that point.
