There are no direct flights from Cuba to the USA on major airlines at this time. There are, however, fligts from Cuba to Jamaica and Jamaica to the USA. Direct flights between the US and Cuba are by charter air. There is a listing of charter companies authorized to fly between the two available online:

There have been rumors about normalization of relations but no change at this time which would allow mass transit between the two. Check with the US Embassy to see if they will allow you to immigrate on your passport using a charter air. You may also go through increased scrutiny at immigration/customs having come from a country which is not considered friendly by the US Department of Homeland Security.

Not directly unless you have special permission from the U.S. Treasury Department's Office of Foreign Assets Control (OFAC). You can try applying but I doubt you'd get permission.

Many Americans travel to another country, such as Mexico, and then get a flight to Cuba from there. The Cuban government has no problem letting American tourists in. The US Government considers this to be illegal, and if you return to the US and the US government finds out you've been to Cuba, you could be fined several thousand dollars. But this isn't really a common occurence, although it does happen especially if one is stupid enough to try to bring anything from Cuba back to the US.

Absolutely not, if you want to go to America via Cuba you have to fly to Canada first then to America, thankfully Cuba is one country the Americans haven't ruined as yet.

OK I think you might have to fly through a third country. I would not have thought that Cuba would make it very easy for its citizens to leave to go the USA in case they end up defecting. So I would have thought you'd need a special visa, or you'd have to go through a third country. See website below:
