"The sound of one hand clapping" is a koan. Koans make no literal sense. That's why the Zen Buddhists use them for meditation. Doing so under appropriate guidance breaks down the rational thought process in such manner that is conducive to the Zen path.
Obvioulsy, one hand does not clap alone. There is no sound, unless you hallucinate.

One hand claps against someone else's face > an ugly sound; that almost never achieves the desired result

One hand cannot clap. "Clap" is an onomatopoeia for the sound of two hands clapping.

Yes, Grasshopper, there is sound. If you do not hear the sound, you are not completely in touch with yourself and your surroundings.
For Pete's sake, get new batteries for your hearing aid!

The sound is very similar, almost exactly like, that of an appreciative sigh.
