when it rains, worms often crawl out of the dirt/lawns and end up on the concrete, where if left, they dry up and die! so if we throw them back onto the lawn where they left from...are we actually saving them?

This is possible, the reason they crawl out is because the water concentration in the ground becomes too high, and their tunnels are flooded, causing a lack of oxygen.
So you might be saving them, returning creatures to their habitat is always a good way to rehabilitate them.
This could make a good idea for a t-shirt: instead of 'saving the whales' you are 'saving the worms'!
It is good to know that some people still care about even the weakest and least appreciated of creatures!
