Can't think of any at the moment, but just to correct a previous answer -
Jill Masterson (Shirley Eaton) dies of "skin suffocation" by being coated in gold, a complication that, despite popular belief, has no basis in fact. The belief depends on the incorrect supposition that respiration occurs, at least in part, through the skin, a fallacy that has been discredited in scientific circles since the Renaissance. Despite periodic debunking in the popular media (especially noteworthy is a 1978 column of the syndicated newspaper feature, "The Straight Dope", and a 2003 episode of the Discovery Channel series, "MythBusters" (2003)), a widespread belief in the myth of "skin suffocation" still exists. The myth was further bolstered by rumors that Shirley Eaton had actually died on set from asphyxiation due to the gold paint.

Whew. It's been 40+ years since I saw it on TV, but there was a movie shot in the 1920's, a black and white silent epic which had a lot of extras and actors playing Russian Cossacks or other horse riders of the appropriate era in it.
The battle scenes involved hundreds if not thousands of extras for the battle scenes.
One of the horsemen in one battle scene took a swipe at another actor with a real sword...and decapitated him.
I don't know, and probably never knew, the name of the film. Certainly not the actor's name.
It's one of those early films they show in film school sometimes.
A lot of horses in this film, and in many others of the era, wound up with broken legs and had to be put down, as well, since they used trip wires to cause the horses to fall down in those days. Unlike the humans, the horses had no choice in the matter...

Raoul Julia died while filming a project in Mexico. I am not sure if it was during the filming of "Street Fighter The Movie" or was a new project started before "Street Fighter" was released.
There was no charriot race in "Sparticus". "Ben Hur" movie where the stuntman died filming the race.

Not really a tragic film making death per say, but you had one of the midgets in wizard of oz that hung himself during filming.
And not related to filming either, but John Candy died on set, as well as John Ridder?(Jack from threes company) died during filming of his new sitcom, and of course Steve Irwin.
