Does one even need a high efficiency oil filter for the car? If an OEM filter is rated at 40um (for example) how many hundreds of thousands of miles would a high efficiency filter gain? (rated at 15um? for example)

Those so called "high efficiency" filters are used only when the car is brand new. As the engine is breaking in, the metals inside are seating in and falling in the oil. Those "high efficiency" filters traps most of those particles so it doesn't circulate around the engine. After the break-in period, you can just get a normal Fram oil filter.

It's just a marketing thing to get you to buy and pay more for "x" brand or type of filter

You can stick with the OEM ones. No more and no less.
