Animes for older age group of audience (young adults) will reveal the nipples. It does not neccesarily mean hentai is the only genre to reveal the stuff, but some ecchi would have them.
Animes with the main target audience of age 16 and below would have mild nudity, but excluded nipples. Those animes meant for above the categorized age would reveal so.
Some reason for your question is that, the anime produced after the manga, may not have the intention to excite sexual content as it may stray from it's actual genre, but only 'suggests'. It's sort of similar a bit like how gravure idols do their work. That is one of the way to widen up the target audience gap for the anime compared to the manga.
Another reason is most probably a marketing strategy. Those who've seen anime most of the time are "director's-cut", and those who wish to view the "extended version" can hope to view it in the manga version. That's one way to promote another related product. Hope that helps.

The censorship laws in Japan are such that you can show basically everything but the nipples and the crotch, so artists are free to show the quasi-nudity with the breasts exposed but minus the nipples for effect.
If you notice the more adults mangas out there such as Sakura Diaries or the more hentai stuff, the nipples are drawn in, because it's targeted towards a more adult audience and it's usually labeled as such on the covers.

A lot of them do. They don't if you are watching the English dub/editted version because of censorship.
Claymore, R.O.D. the TV, and Blue Gender immediately come to mind that show nudity and nipplpes (but aren't considered hentai by any means).

Though I agree that unedited anime would show you this but these types are often restricted to ages 15+.
