It is quite a good way to spread STDs in general and syphilis and gonorrhoea are quite commonly anally spread. If there are any skin breaks then HIV and hepatitis C would be a risk. Finally ignoring STDs the anus has its own normal bugs, including E. Coli which produces food poisoning!

In summary this is not exactly safe sex!

I'm not an expert but I don't think there is necessarily a AIDS risk involved in "rimming". As long as there is no blood involved. As the other answerer said it can be unsanitary but the real risk is hepatitis. If you like doing it go ahead but you might want to consider going down to the free clinic or health unit and ask for the Hep A & B vacine just to be on the safe side. Personally I love doing that and find it to be a real turn on! I've had the shots.

It's called rimming. Good hygiene is necessary. The biggest risk is hepatitis. If you're going to engage in rimming, then I highly recommend getting vaccinated. Not a big deal to do. It's like a 3 shot series. Can get them at free clinics.
I've been rimming for about 20 years. Love it and the chicks really get off on it. Of course, I enjoy receiving too. If you haven't tried it, you're really missing out on an awesome stimulation. And no, if you're a guy that likes getting rimmed, it does not mean you're gay. In fact, it's becoming more common in str8 porn now adays. Check it out!

Not AIDS but hepatitis is the disease you might catch while doing this. I suggest that you clean both yours and your partner's anal tract out very THOUROUGHLY and wash the immediate area in the shower/

Have you ever heard of fecal-oral infections?
When I was in med school, one of the first things we ever learned was, "don't eat sh**"
For the sexually transmitted diseases, the transmission rates are similar as for conventional sex.
No, it can never be "safe" as far as STDs, and plus, you get the added risk of ingesting fecal material and getting serious diarrhea or hepatitis or something.

Not very risky re: HIV. hepatitis A is transmitted via fecal-oral route though. As with any sexual act...best to know your partner well. You could also use a latex rubber dam..not as fun, but safer.

You will pick up E Coli and get a nasty case of sickness and diarrhoea.......I doubt that you'd catch HIV though. This is contracted via blood, so unless you were sucking on his piles, there'd be not much chance of contracting it this way!

Use a dental dam or another barrier when rimming someone. You can pass germs and get very sick if you do this unprotected. Good Luck.

If you absolutely must then use dental dams to reduce the risk of oral-fecal infections.
