Well I just had my 34th week exame yesterday and it so happens that she was teaching a intern on staff and she explained to him that when they pinch down near the lower part of your pelvic, fromt he outside they can feel a destinct lump in your pelvic bone, and then tells them then the head is down, so if you feel them grabbing just above your pelvic bone this is why. Another way she said that they can tell is the lump that is often at the top of your belly, is more prominante and sticks out harder if its the head, and is not so prominante if its the bum.

you can feel the head down even without an internal a nurse and Dr can tell the position just by feeling the stomachs surface
