Yes. They have to pay you. You did the work that they contracted you for, and they have to. Don't get me wrong, some employers can be mean about it, and just decide to string you along. If that's the case, get the labor board in on it.

Yes, and it has to be made availible to you on your normal payday UNLESS they fired you. They have to also keep your insurance in force until the end of the month that you left....a well kept secret is that employeers pay the insurance premium at the START of the month, before they have gotten your part of the payment from your paycheck. The insurance is still in effect until the end of that month.

Yes, but they don't have to pay you for the days you were supposed to work and didn't. If you quit, they also don't have to pay you for the day you quit.

They can pay you on the next, regular payday, but some states require payment the same day, but very, very few states do that.

In the US, you must be paid for all hours worked through the date of your termination. In some states, you must also be paid for accrued vacation or personal time off.
