you've got it in the right order. do birth certificate. county certificate of live birth certified copy. you can order online in most places. if not currency exchanges/ court houses can help.
social security card comes next.
lastly, you call your doctors office and get records.
my doctors have a passport sized insert with the vaccination records for your passport. i keep mine in there.

You have to contact the Bureau of Vital Statistics for the birth certificate. Social Security Office for a new card. The shot record would come from your local health department or family physician. ID is either a drivers license or state issued ID

birth certificate ... usually, no id is required to get the copy
i think SS card is next, but i'm not sure
then ID ... you'll likely need witnesses (with ID) who know you to testify to your identity
shot record ... that should be fairly simple ... you write to the physician(s) involved and ask them to send same to your current physician.

Consult a lawyer for an affidavit, your old birth cert. then he will instruction before filing same in Court of first instance after paying big fee.
