In order to subtitle anime you need a few things.
1: The original (RAW) file that you will apply the subtitle to. You can get these from sites such as http://www.tokyotosho.com/ .

2: A subtitle creation program such as Aegisub, found here: http://www.malakith.net/aegiwiki/Main_Page
To make the subtitle, a person listens to the original raw file, and translates what is being said. After they get the script, they then use timecodes to tell the sub at exactly what time each line needs to be displayed, for how long, what position, and describe the font style.

3: A program to join the raw and the subtile together in either a hard (subtitle become a part of the video and can only be watched with it on) or a soft (subtitle is combined with the video in a container such as mkv or avi, can be turned off and on, and can be removed) format. You could use something like VirtualDub with VSFilter to accomplish this. Although VirtualDub hasn't been updated in quite some time...

In conclusion, it is no easy task to make a fansub, and you (or someone you know) would have to know japanese in order to translate what the people are saying.
