Three things:
- Read a lot, especially non fiction (which makes you exercise your thought processes). This is No 1 in importance. Don't be afraid to consult a dictionary - it's amazing how many words do not mean exactly what we think.
- Write a lot; letters to your friends, essays; keep a diary (these exercises sharpen up your vocabulary and your logical processes). Use a thesaurus, to help express your thoughts. Computers make it so easy to write essays, record your thoughts, etc; but don't disregard the simple pen and paper.
- Practice a lot - especially with mathematice, and other exercises , such as puzzles and crosswords (these sharpen up your mental agility).

Knowledge will come along once you put more concentrated time into studying, even when you don't have a test. I know this is just a simple answer but it is the best I can think of at the moment. Whatever your "sore" subject/s are you need to practice- like in math practice several different problems, maybe if you don't do well studying on your own get someone (that makes good grades preferably) to study with you.

Exercise the body and read more. It's very simple, people are designed to function with as many choices as we have in our society. Keeping it simple will help you overcome many of the stresses that you feel.

schedule your time wisely to get assignments done.
challenge yourself with slightly difficult classes.
get a study partner who wont get you off-task.
read one of those self-help books; sure theyre corny but htey help if your follow them!
