What happens if there is a mistake on my child performers permit?

I sent my papers in for my child performer's permit, but when it says
Parent/Guardian:
It has my name instead of my mothers. What do I do now?

I sent in the papers, but when they sent me the permit, they made a mistake, and where it says Parent/Guardian they wote my name instead of my mothers. what now?

Call and report immediatly and then the rep will tell you the procedure from there

I would contact the agency that issued the permit. They most likely mixed up the names. I would contact them as soon as possible so you don't have any problems down the line. You might want to have your mother contact them, since you are still a minor.
