It has lost all its leaves and the smaller branches are getting extremely brittle. The larger branches seem to be cool to the touch like they still have moisture in them. The trunk has bark that is splitting and coming apart.

Willows (Salix) are generally weak-wooded trees, and subject to an endless litany of insect and disease problems. They generally have a shorter life span than hard wood trees. I would throw out there that your tree might have canker. That will cause fissions and cracks in the bark. You might also get bulbous 'swollen' areas or sunken 'bruised' looking areas around the cracks. There is no cure for this disease, though trees can live with it for years. Fertilize the tree and keep it moist. Three years old is very young, most willows don't get these serious problems until they are much older.

It could be that it is not getting enough water and /or getting too much sun. Willow roots seek out water so never plant near septic or water lines in yard. Have you used any weed killers that could have drifted or leached to the tree?

Maybe the soil needs ammending? Add some fertilized soil around the base and water it daily for a few weeks. It wouldn't hurt to spray it with a fungicide/bug killer spray. They have some at Lowe's...can't recall the name though.
