On monday I had a positive OPK test and same on Tuesday, I also went in for a vaginal ultrasound on Tuesday. Dr. said there was a healthy folicle in there and I should notice a spike in my temps in the next 24-36 hours. anyhow, Wednesday had another positive OPK and VERY VERY hard and sore nipples alll day long, Today another positve OPK and still sore nipples but no spike in temps.. Is this a normal symptom to have when ovulating (sore nipples) and also what is the deal with still no spike in my temp and still having positive OPK's is this all normal. This is only my 2nd cycle of clomid and I did not ovulate at all last month, so still not sure how all this works. Thanks for all your help!

If you have a pos opk for more than 3 consecutive days then it could mean there is a problem. Have you taken OPK's in the past and gotten accurate results?
But with the LH still surging and the temps not shifted then you can safely assume you haven't O'd yet. You really need to contact your doctor and let them know the OPK's have all been positive and no temp shift. They will probably want you to come back in for another ultrasound and some blood work.
Also, one other note, your Pre-O temps are rather high, bring this to your drs attention, and also having high Pre-O temps can make it really difficult to tell when you actually ovulated.
