Hello?? And you are in a helping profession?
If you are that busy, you shouldn't waste your time with that.
You need a policy, for all your patients, that you post in your office, saying that you will charge for "no show" appointments, or appointments that are changed with less than 24 hour notice.
Your time is valuable, and money is the currency that we use to exchange for our time.
Take a look at your anger issues.

I somehow doubt that you really are in a caring profession.
If you are, then you do not insult or anger your patients.
Simple send them a bill.
Before making future appointments, ask for payment in advance from those who don't turn up.

You should know that anything you do or say will not change them, or even affect them. They probably won't even open the letter. Just bill them for a missed appointment, and find some other patients.

Very unprofessional
you do that and you won't have to worry about any new patients, cause people talk.
you need to put a 24 hr notice regulation on your appt card stating that if not cancelled within 24 hrs there will be a charge of $_____
which will be paid before another appt is booked...

Wow, you sure are a compassionate and caring physician, aren't you?
Sure, you go right ahead and do that. Pretty soon you'll have so much of your valuable professional time on your hands, you'll be able to do very important things in your office. Like twiddle your thumbs or surf the internet all day. Maybe even make some money on Ebay selling all your office supplies and furniture that you won't need.

Don't!!! I know it isn't what you REALLY want to do but grin and bear it and be REALLY nice to them. write a letter saying that you are sorry they couldn't make the appointment and that you look foreward to seeing them again soon. It'll piss them off more that it riles you.
Plus, it'll make for future clients if you're not nasty to the ones that piss you off.
I'm a holistic therapist so i know!!! You give them abuse...they don't recommend sweetie.

Sounds to me like the counselor needs a few counseling sessions of his own to help him deal with his anger issues. Drop the idea; it isn't professional and will only ruin your practice.

You don't. Those actions to which you speak are very unprofessional. The policy at our office is if you cancel or no show more than three times we elect to no longer see you or treat you, you must find another doctor. Some doctors also will charge you for missing appoints.

Dear sir/madam,
I hope for your well being.
I regret to inform you that you have one more chance to prove to me that you are a reliable patient, as you have not been attending your appointments regularly.
If you do not attend your next appointment on the date below, you will be dropped from my patients list.
Kind regards,
(.....your name here.....)
Note: If you want this to work, please do not load it up with swear words.
