HTML code for Comment page needed?

I need a HTML code for comments box to be created . Also, I need the comments to be submitted to an email address.
This table should be:525px wide, Cell Pad: 0, Cell Space: 2, Align: Default . If you can not get the size any comment space box HTML code will due.

For comments to be submitted to an email address, go fetch a userid (account) at http://www.jotform.com
There, build a simple form (they, or he, as it turns out, have/has a template for a simple "contact me" form which sends a "comment" to an email address you specify. Build the form on the interactive editor. Copy the supplied source html to your web page. Restyle it so as to make the comment field your desired width (525, for example). You can see, in the supplied html code, how the supplied table is styled. Use it accordingly.
