As long as your ground is level then you will be fine. I laid my front walk that way and it hasn't moved. However, I did plant Liriope grass alone the sides and they act as an anchor for the pavers.

My walk way has survived 7 years. I did not lay it on the grass. I laid it on soil.

Basically, that's all I did. I used a lawn roller filled with water (to add more weight) and smoothed/compacted the ground, and layed 9x9 pavers. It's been like that for almost 3 years now.
I would say to put down some sheets of the black landscaping cloth to keep weeds from growing up between the pavers.

It depends on the type of soil you're on, but the reason you lay the rocks and sand is to stabilize the ground under your patio and allow for good drainage. If you don't lay a proper foundation, your patio could tilt or crack. Do it right the first time, or your patio will be likely be ruined in a few years.

Just be sure to remove the sod when you level the ground. My sister had one put in and she thought it would be ok to leave the sod since we put sand down. Some of her pavers have sunk since we didn't remove the sod.

depends on how settled your yard is. if its "old dirt", it shouldn't settle too much. if your yard is only a few years since it was graded, you may want to lay a layer of sand down.
