Pain is one way to tell but the sign that can be a strong indicator of a fibroid is bleeding that continues for quite some time and be heavy. One way they can find out if it's a fibroid is by doing an ultrasound to see what is going on with the uterus.

The only way to know for sure if you do have a fibroid is via ultrasound.
I have a hx of fibroids and polyps, have had both removed via hysteroscopy with D&C. My symptoms were heavy bleeding and severe cramping.
