Maybe the doc thinks we'll be embarrassed when he sees us undressing.

To give the patient some privacy while she's undressing. Then when they are sure she's ready they will knock on the door. If the patient is ready the doctor will then come in to the exam room.

Good point, cos he probably gonna see you naked anyway. maybe its because he does not want to make it think that either parties are making sexual gestures at the other.
