The largest use to be in Oklahoma.... but is now in Moscow Russia.
Not sure about the smallest.... That might be Chandler Oklahoma.... Inside the small Wal-Mart there.

When we visited China in 2005, I found some McDonald's kiosks that only offered soft-serve ice cream and pre-made sundaes. In a photograph that I took, it looks like the menu board offers vanilla, chocolate, chocolate dipped cones and three flavors of sundaes. The kiosk was large enough for two employees inside (barely).

I can tell you that the very first McDonalds is still standing as a museum in Desplaines Illinois. They will open it for tours as well.
