as far as i know, this was only done in college basketball, and was known as the lew alcindor rule - it was banned for ten years because kareem abdul-jabbar was too tall. They finally let it go as more dunking-capable players started playing. As to the rules effects: i think that it the game and strategy would obviously have looked very different to a viewer today - but dunking was banned because few could do it in college and it (dunking itself that is) changed the way people thought the game should look at the time. Basketball was designed without dunking in mind.... so although it looks cool, many people felt that it was changing the game tremendously (as indeed it has, for better or worse).

I think they banned dunking in college game in the late 60's during the time of Lew Alcindor (Kareem Abdul-Jabbar, of course). Kareem, at 7'2", was so tall and strong, and he was dominating the game the way Wilt did that college officials figured, rather stupidly, that banning the slam would level out the playing field. The result, Kareem developed an even deadlier weapon - the skyhook. And the rest was history.

College basketball did. They banned it because Lew Alcindor aka Kareem Abdul Jabbar dunks too much and seems to be an unfair advantage for others(if that was the craziest excuse for banning dunking but it's true).
Well, it made Kareem Abdul Jabbar a deadlier scorer since that's when he learned to use the legendary hook shot that made him all-time leading scorer in NBA history.

Beginning with the 1976-77: Dunking is reinstated
And yes, Alcindor was the one and only reason for the ban. And it made him an even more formidable offensive weapon.

The banned it in college basketball in the 60s and early 70s because when Kareem was going to college they thought it would be unfair to other teams,they were afraid that he would average 50pts a game but he still did he average 35....
