The dead guy lying in the middle of the floor. All through the movie you think he's really just a dead guy and nobody pays much attention, and then he winds up being the bad guy...he was alive the whole time, just lying there on the floor. Major twist...normally I'm not a horror fan, but I have to give kudos to the writers for that sweet twist.

the guy in the white room in a pool of blood is the bad guy. he wasn't really dead. and he was wearing a mask. when he took off the mask he was the guy lying in the hospital bed and the doctor was talking about him to the nurses? "the very interesting man"
