If you live in Southern California, you could check out Flame Broiler (http://www.fbroiler.com/). Also, chicken places that don't fry their chicken (like El Pollo Loco, http://www.elpolloloco.com/) and Boston Market (http://www.bostonmarket.com/home) will usually have a nice selection of reasonably healthy choices.

Many Taco Bell locations will let you ask for your items to be prepared "Fresco Style," which means they'll substitute any sauce or cheese in the item for an all-vegetable salsa, sometimes dramatically lowering the fat content.

Generally speaking, if you avoid any *fried* foods and ask for your food to be made without cheese and without mayonnaise (or any other creamy sauce that's likely to be mayonaise based), you'll dramatically reduce the fat content. Also, you should be aware that burger places that grill or flame broil their burgers (like Carl's Jr/Hardees and Burger King) will usually have lower fat content than places that fry their burgers.

This site is rather great for finding out what you can order at restaurants near you, including fast food restaurants, to get a reasonably healthy meal:

Its hard to think of a healthy fast food place besides subway. However, you can always pick something semi healthy at fast food joints like a salad, baked potato, grilled chicken.

Despite its name, Fat Burger is pretty good in terms of not being super fried. Quiznos is delicious and has healthy options. Oooh, I think I'm going to get Quiznos for lunch.
