You need to put down 2" of 57 stone down then some plastic over it then the cement this will help keep moisture down. If the ground under the shed gets wet it will keep your cement floor high in humidity which eventually can cause things to rust that's why I wouldn't use sand it holds moisture. I just put a shed in last year.

the only thing you need sand or rock for is to level any wierd spots out to keep your 4" consistency. your not making a very big shed so dont try to over-engineer anything.you do need to dig a footing though around the outside. as far as moisture goes you shouldnt need to worry about that that. its a shed with a roof not a dining room.

Depends on soil condition and location.
The sand or gravel layer can help prevent frost heaves and allow the slab to settle a bit without cracking.
