To align is to be able to let go of the wheel for 3 seconds and it still stays straight and doesn't pull. To rotate tires is to change the wear of the tired but might change the alignment or balance of the tires. Which would make the rotation worse than when you started if you don't realign and rebalance. So, I repeat, yes, do them all at the same time, unless you can still let go for 3 seconds with no pull, then maybe you are still good until the next rotation.

Have your tires Rotated And Balanced every other oil change(6000 miles).
I would have the alignment checked once a year(12000-15000 miles) as a preventative measure, this way you catch a problem before it becomes serious.
If you notice the car pulling to one direction or see unually tire wear, this is an indication of an alignment problem or bad suspension part.

Alignment no, unless you drive on roads that has alot of potholes. Balance and rotated are usually free from the place where you bought your tires. At least mine are. When I ever buy a set of tires, they always have free rotation and balancing for the life of the tires.

I always have the balance checked on my tires everytime i have them off. I personally only align my tires about every 50 thousand miles or when i notice uneven tread wear
